HR - How To Request a Name Change

As an employee, you can access data through self-service, such as your name, address, and phone information. Although not all data stored in your profile can be edited, you can request a change to your name via the personal information screen.

Before requesting a name change, it's crucial to consider some important factors:

  • Legal Name Changes:   Your Legal Name must match the name listed on your Social Security Card.   Be sure to attach a copy of your Social Security Card when requesting a change to your Legal Name Information.
  • Once HR receives your Request:  HR will review it for approval, and you will receive a notification in your work email address when it is approved. Your new name will be updated in all Infor/COA-related systems within 48 hours of notification.
  • Upon completion of your Request:  Your email address will be updated in Outlook to match your new legal name.  If your name change impacts your benefits, please refer to the Blue River website for more information on Benefits.

From Self Service: 

1. Locate the "Profile" Section

2. Select "View More".

3. Select "Change Name".


4.  Complete the required fields on the "Request to Change Name" form.

a. Select the date that you want the change to take effect in the *Effective Date" field.

b. Select the reason for your request in the *Reason" field.

c. You may enter a title in the "Title" field where appropriate, for example, Ms. Mrs. or Ms.   This field is optional and does not require entry. 

d. Enter your First, Middle, and Last Name as displayed on your Social Security Card.

e. The Preferred Name fields contain your Preferred First Name, Preferred Last Name.  You may enter a nickname in the Preferred First Name field if you have one.  

NOTE: (Please ensure the nickname is appropriate for a professional setting, as it will be displayed in COA/Infor-related systems).



    f. Upload documentation for your Name Change Request. 

    NOTE: If the name change is a legal change, please attach a copy of your Social Security Card to the request.


    g. Enter additional information relevant to your name change request in the Comment Box. 


    h. Select "Submit" to save your changes and submit your request to HR for approval.
