How To Request an Affiliate Account ID

If you are working with a nonemployee who will be working at our facilities or accessing our network, they must be registered in Infor to obtain a CHS account number.  

This process applies to adding or reactivating user accounts for most non-employees, including:

  • research assistants, 
  • non-MD/DO student rotations, 
  • local temps, 
  • UAB staff, 
  • consultants,
  • and outsourced services such as billing, 
  • scheduling, 
  • or other patient support.

This process does not include any medical staff/providers as they are managed by Medical Staff Services. All new providers should register at http://iteducation.chsys.org/signup/.

  • Medical Student rotations
  • Residents
  • MD/DO Fellows
  • CRNPs
  • CRNAs
  • or PAs
  • Maintenance, delivery, and repair vendors who need badge access are managed through facilities in a different process.

We highly recommend initiating the affiliate account request a week prior to the first date the affiliate will onboard. Once the user profile is created in Infor, the account information will automatically create a network account, email account (if necessary), Children's University account, security badge, Everbridge emergency notifications, Lucidoc, and other systems. Please note that any affiliate requests submitted within a week of your arrival may cause delays in system access, training access, parking, and security badge issuance.

To ensure a positive onboarding experience, please complete the Non-Employee Affiliate Account Request a week prior to the arrival of the affiliate.  If the affiliate had a COA network account in the past but has been gone for some time, a new form is needed.  To extend the time on an existing/recent affiliate account, please email [email protected] to request that the profile be updated.

After submitting the Affiliate Form, you will need to complete additional forms for parking/security badge, remote network access, and requesting access to clinical systems, if appropriate to the role. These forms will be emailed to you at the address you provided. It's important to keep in mind that accessing clinical systems may require completing training beforehand to qualify for specific roles and systems.

It is important to ensure that the COA coordinator who will work closely with the new affiliate is provided. Once complete, the affiliate will receive an email with the assigned CHS number a few days prior to arrival.  The affiliate can refer to the instructions provided in the "Affiliate Account Resources" article to register their new accounts and access necessary system resources.  Access to specific data systems must be submitted to IT using the Request For Access (RFA) process by the coordinator, using the assigned CHS number.
